This treatment plan sheet is basic but it easily outlines results of an evaluation, goals, and treatment frequency/duration in ONE sheet for parents. Concise information is key!
I LOVE LOVE LOVE my session log sheet. Again, it is simple but a HUGE time saver and organizer. I can organize all the sessions and keep track of payments in just a few seconds. This form includes drop-down menus and other time saving features.
This spreadsheet is another time saving feature. I realized by organizing insurance info in one spot, billing insurance takes seconds instead of minutes. Again, every minute counts in private practice :)
This one is my favorite out of the packet. Using a spreadsheet and the duplicate feature, I can record updates on goals, update goals as needed, easily print notes for insurance, and of course, track progress quickly!
Some patients prefer to pay cash on a monthly basis or need a receipt for their records. This quick form is a great too!
